Powers (no more than 150 words): Blythe is possessed by a demon calling itself Pestilence, the Horseman. With Pestilence, Blythe can induce any sort of illness in living organisms she can directly touch, as long as she gives consent to Pestilence to infect the living thing. Pestilence can also directly control Blythe's neurotransmitters to flood her brain with any signal She chooses, can refuse to allow Blythe to infect something, and can cause pain and organ failure within Blythe if she doesn't give Her what She wants.
Personality
What is your character most afraid of? How are they defined by this fear? (100-300 words) Blythe will say that she's most afraid of being a subpar vessel for Pestilence, of doing anything that would make Pestilence leave her. In a way, she's not lying, because that would pave the way for something that she fears even more than that. She doesn't want to die without knowing that she matters to somebody. She's terrified of dying alone, unloved—hell, even unhated. She doesn't want her entire existence to all come to nothing, and she's been terrified of that ever since she nearly bled out from a head wound on the floor of her family's own kitchen as a child. With Pestilence, she knows for certain that she can at least do something to make Her happy and leave a mark on Her. She'll do all she can to make herself the perfect host for this demon who does not have her best interests anywhere in Her heart, and if she tries hard enough, maybe it'll all matter. Maybe she'll matter.
What is your character's greatest desire? How are they defined by this desire? (100-300 words) Blythe wants to change the world. She grew up to two parents who took every opportunity they could to chase every money-making scheme possible, and part of her still believes that she was only born so they could claim tax benefits. She was forced to raise herself almost completely and resents not her parents, but the system they lived in for making them turn to this. She wants more than anything else to dismantle the roots of capitalism and usher in a society where children do not go hungry, where parents do not have to leave their families abandoned to make ends meet, and where there is no longer any incentive to "get rich quick" since the idea has been abolished entirely.
This desire is what led her to summon Pestilence, and now that she has been granted power, she is pushing through with her desire to change the world in the only way she knows how: Killing warmongers, fundamentalists, and the ultra-rich, all of whom prey on the lives and livelihoods of others in the name of prolonging their own lives and legacies. With the arrival of Pestilence, Blythe's core desire has remained the same, but it has been plastered over with ones that are less organic and more insidious. The most prominent of these, and the only thing that could be considered close to supplanting her greatest desire, is to continue to be a good vessel for Pestilence.
It ties into her greatest fear as described above, but she can act directly against her own self-preservation if there's a chance that Pestilence may leave her. Pestilence's goal is to "maintain the balance between life and death", but since She's the only one who knows what this balance looks like, She can tell Blythe basically anything and Blythe will have no choice but to believe Her.
How will not having memories change your character? What core personality traits, if any, will change? How? (200-300 words) Without knowing who Pestilence is, while still being aware of her abilities and not being afraid to use them if necessary, Blythe will need to adjust in a pretty serious way. She won't be able to recall the thing that's been keeping her in line and controlling her relationships for the past twelve years of her life, so she'll likely feel incomplete in a way that doesn't seem right at all. There won't be any way to rectify that feeling, leading to her withdrawing more and distancing herself while she tries very, very hard to figure out what's missing.
Of course, Pestilence will still be there even though She's locked away, which will lead to a lot more confusion on Blythe's part as well. She'll be even more sickly than she usually is, complaining of aches and pains that she can't ever find the cause of, and her emotional state will be far less stable than normal as Pestilence beats against the bars of Her cage and floods Blythe's brain with hormones.
In short, Blythe will transform from a woman trapped in an abusive relationship to a slowly recovering woman with trauma that she doesn't even know how to talk about. So many of her formative years have been based on things that Pestilence has decided for her, so she'll feel uncertain about her identity, her gender, her purpose in life... she'll be more lost than anyone has ever been. Whether she knows it or not, she'll be focusing on recapturing the years of her life that she wishes she had been fully cognizant for.

Major memory: She has killed at least a double digit amount of people using these abilities, and remembers not being repulsed by the act. In fact, she remembers feeling satisfied and at least a little thrilled over the course of these killings, because they were people who controlled too much of the world with their wealth and power. They were mercy killings for the planet.

Major memory: She fell off a counter and nearly bled out on the floor of her kitchen as a child. She knew that her parents weren't there and she couldn't call out for help from anyone else. She was utterly convinced she would die there, alone and forgotten.

Major memory: Blythe has been sick for almost as long as she can remember. She doesn't know the source of her chronic illness(es?), but she'll just get these debilitating pains inside of her every so often. Her brain feels imbalanced, her body is frailer than it should be for someone of her age, and she has a persistent, bloody cough that she's just learned to deal with.
